Request to join A Healing Arts Network
Provider application requires verification of professional license and insurance. Once provider is verified an ICA- Independent Contract Agreement will be forwarded to you to review and sign. Please provide the following information:
Submit a copy of your State ID, professional license, liability insurance and photos of your required working equipment and products.
